This is the vanity card of Irish artist, film and game maker David OReilly, used once on the Adventure Time episode "A Glitch is a Glitch", which he wrote, directed, and animated himself.

Logo: On a gray background, we see the rounded square with a line across it. Then, a circle appears with a duck inside, and the words "dAVId O'ReILEY" in some strange font appear.
FX/SFX: The circle, the duck, and the words appearing.
Music/Sounds: A beep and a quick synth whirr.
Availability: Seen on the Adventure Time episode "A Glitch is a Glitch".
