Visuals: On a white background, there is the green triangles from the American Vision logo, minus the name. In front of it is the following text in three colors scrolling up:

Technique: Computer animation.

Audio: A cheap dramatic synth fanfare, which is "Cliffhanger - De Wolfe Music" (that continues from the logo).
